Microsoft Forms Overview
Forms gives instructors the ability to create surveys, quizzes, and polls for use both in the classroom and for online courses. The user-friendly interface and real-time analytics make assessing understanding, collecting feedback, and engaging students efficient and reliable.
Forms Availability
Forms is available to everyone in the UIW community with access to Microsoft 365 applications in Cardinal Apps,
- Accessed through any web browser and integrated within other MS365 applications.
- Windows devices have a downloadable desktop application option. There is no downloadable macOS version.
- Sign in with your UIW email and password ensures you can access all the features.
- Tech support is provided by the UIW Help Desk.
Forms Considerations
- The performance of the Forms web application is impacted by the browser you use to access it.
- For the best possible experience, use Microsoft Edge or Google Chrome. If you encounter unexpected behaviors, clear your browsing history or change browsers.
- Formative assessments to monitor student progress and adjust instruction works well in Forms due to instant analytics when compared to the time it takes Canvas Quizzes to create reports.
- Creating anonymous surveys to collect feedback from students is simpler than using more robust products like Qualtrics.
- Practice mode allows students to attempt a question multiple times before moving on, which is a feature unique to Forms, and best for team-based learning.
- Instructors can export results and use the spreadsheet data to import grades into Canvas.
- It is integrated for use with PowerPoint, MS Teams, Power Automate, and Stream.
- Use the embed code or link options to add inside Canvas courses.

Can I add a poll to an MS Teams chat or channel?
Yes. If you do not want to share a Forms link or add a Form tab, you can use the app called Polls.
Can someone without a UIW account take my survey or quiz?
Yes. Forms authors can toggle their settings to let people outside of UIW respond. However, these responses will be submitted anonymously. If you want to see who has taken your survey or quiz, require respondents to fill in their names as part of your questionnaire.
Can I translate my Form into another language?
No. Not directly. However, most browsers have built-in translation features. Select the browser you're using below, and then follow the instructions to translate form content into a different language:
Is the Immersive Reader available in Forms?
Yes. Customize the way text looks for easier reading, or have text read aloud with Immersive Reader. To launch Immersive Reader in Forms:
Open the form and select ... then choose Enable Immersive Reader.
Hover over any portion of text in the form to bring up the Immersive Reader icon
Select the Immersive Reader icon Immersive Reader will launch as a full-screen experience.
